Hospital & Transitional Care

When you or a loved one needs more time to heal after a hospital stay, our Transitional Care program lets you recover right here in Big Sandy β€” close to family, close to home. We provide skilled nursing, rehabilitation, and personalized care planning for patients who no longer need acute care but aren't quite ready to go home.

This care is often covered under Medicare as part of our swing bed program β€” a designation that allows Critical Access Hospitals like BSMC to provide transitional, skilled-nursing-level care using the same beds used for acute patients. Our staff will help determine eligibility and walk you through every step.

Skilled nursing β€” Around-the-clock nursing care including medication management, wound and IV care, vital sign monitoring, and chronic disease support.

Rehabilitation β€” Physical therapy to rebuild strength, mobility, and independence β€” with personalized goals for getting back home. Occupational therapy services are available through coordinated arrangements when needed for your care plan.

Discharge planning β€” Care coordination, home safety planning, family and caregiver education, and follow-up scheduling to ensure a smooth transition home.

Comfort & whole-person care β€” Nutritional support, daily activities, emotional well-being, and family involvement β€” because recovery is more than medical.
